Coochbehar District Recruitment December 2013 - 80 ASHA Worker

Coochbehar District Health & Family Welfare Samity are invited  applications for recruitment  for the post of “ASHA worker”. Blocks under Mathabhanga Sub –division, Coochbehar. as per the details given below.

Eligibility Criteria
1. Should be a married/ divorced or widowed woman.
2. Should be a resident of the same village for which she will be selected.
3. Should be a citizen of India as defined in part II of the Constitution of India.
4. Should be within the age group of 30-40. In case of SC & ST candidates, the lower age limit may be relaxed to 22 years. Age should be calculated as on 01.12.2013.
B5. Should be Madhyamik appeared or equivalent.
B6. Women candidates who are Madhyamik passed or possessing higher qualification are also eligible - however, in case of candidates possessing higher qualification, only marks obtained in Madhyamik or equivalent examination will be considered.
Selection Criteria
1. The selection would be based on - Marks obtained by the candidate in the Madhyamik or equivalent examination (90%) weightage). - Score in the interview (10%) weightage).
How to Apply :- Interested & Eligible Candidates may Apply. Prescribed  application form available on  below link. The Application form is to be submitted at the respective B.D.O Office of the area/block.  All these two (02) passport size photographs must be self attested by the candidate and also to be attested by any Group-A Officer in such a way that some portion of the signature of the candidate and the officer along with the officerKs Seal, lies on the paper and rest portion of both signatures and seal remains on the photo.
All the requisite documents as prescribed must be tagged with the application and application along with all these documents should be submitted to the respective BDO office by hand for this purpose on or before the closing date. One self-addressed envelope (23 cm X 10 cm) affixed with postage stamp of Rs.5/- (rupees five) only, must be fixed with the filled up application. A candidate may also submit her application in prescribed form through post/registered post which should reach the respective BDO office on or before the closing date and time. Applications received after the closing date and hour shall not be accepted. The Application Form shall be filled in candidates own handwriting in blue/ black ink and signed by her. Should any of the statements made in the application be subsequently found to be false within the knowledge of the candidate, her candidature will be liable to cancellation, and even if selected to the post her service will be liable to be terminated. Willful suppression of any material fact will also be similarly dealt with. Candidates should take particular note that entries in their applications submitted must be made correctly against all the items which will be treated as final & no alteration or addition in this regard will be entertained after submission of the applications. The filled in Application Form should be made complete in all respect. Incomplete application Form shall summarily be rejected without any notice to the candidate or else.  
Documents To Be Submitted Mandatorily By The Applicant
1. The proof of residence (Voter Identity Card/ Ration Card).
2. Attested copy of Mark Sheet of Madhyamik or equivalent examination as applicable. Mark Sheet is to be provided by the candidates even in case of failure in the examination.
